SimmStick™ Design Contest 2000 Competition Entry Form

 

Instructions.

  1. Fill in all of the details on this form.Items marked with a * are essential and must be filled in.

  2. Read the agreement.

  3. Submit this form to your nearest participating dealer ( select from the list below ). Note All Simmstick dealers can provide Simmstick products and support, only selected Dealers are accepting entry forms.

  4. The dealer will email you an acceptance form with your contest entry number on it. Please note that this could take upto 24 hours.

  5. Print out the form, read it and sign it.

  6. Fax or mail the form back to the dealer who accepted your entry. Details will be on the email sent to you by the dealer. DO NOT email this form back to the dealer. Only faxed or mailed copies will be accepted. This signed entry acceptance must be received before your entry will be accepted.

  7. Start designing..

  8. Document design.

  9. Send entry in to SimmStick. Make sure you have faxed or mailed your signed entry form to your dealer.

  1. I have read, understood and will abide by the rules and regulations of the SimmStick Design Contest 2000 as displayed at http://www.simmstick.com/rules.htm and any updates to those rules as made from time to time, http://www.simmstick.com/rules1.htm.
  2. The project I am entering is my own work and that I have the full legal right to exploit the design.
  3. I have been encouraged to seek patents for any unique or proprietary aspects of the design prior to submission.
  4. I retain the the intellectual property rights to my design.
  5. I am assigning the SimmStick Dealers nonexclusive full-publication rights to all material submitted to the contest in both physical and electronic form.
  6. I am assigning the SimmStick Dealers the rights to use my name and/or likeness for any publicity or advertising purposes associated with the contest.
